The original nopple broke and someone tried to ap it to except a threaded nipple?
I would lay a nut over he hole an hit it with the mig gun and weld the tap to the nut and back it out with a wrench .

That's what I do too. Just no extra time at work today. One of our youngins dropping a dmax quadcab off the lift. Lol... oops.
