The oil pump locked up and took out the worm gear. I took apart the oil pump and polished and deburred the ramp and it seems to have fixed the issue.

Click to expand... It was definitely a burr inside that caused it. I could spin it over by hand and it would get caught up at the same place just about every revolution.
