Probably a failed lifter in the displacement-on-demand setup. Pushrod jumps out of the lifter cup and then lifts the valve way too much, valve meets piston and it gets ugly from there. I know 5 people who have had that happen personally

I have never seen that.
I've had failed lifters and bent pushrods.
I've had cylinders drop a valve but that was due to a broken valve spring breaking at a point it lost all tension on the valve.
The GM pushrod V8's are a non interference engine.
