Keep losing sprocket bearing ms461

Good evening Gents,
This is the second time my 461 lost the e clip, sprocket bearing, and retaining washer. I just got done dropping a decent size stem on a removal I did today and was walking towards the tree when I noticed the slack in my chain so I shut the saw off and popped the clutch cover off. All that remained was the plastic cage of the bearing. Is this a common issue? Has anybody else had this problem before, or is it something Im doing wrong?

Ima give ya guessinosis, chain to tight blowing out bearing ,when one side of the bearing gives it essentially prys the Eclip off the crank ...but that's just a guess
