Guess we should also mention chain breakage.
If a chain loop breaks along the bottom of the bar, there will be nothing to pull it into the sprocket; the lower portion will likely drop off, with the top portion being 'pushed' off the top of the bar.
If the chain loop breaks on the top of the bar, the loose end can get 'whiplashed' around the nose of the bar, and towards the operator. Chain catcher, and other protection mentioned above, could really be important in that situation.
Mechanical harvesters also bear the risk of chain links breaking and being thrown at the operator: 'chain shot'.

Chain catchers won't help with chain shot. Lexan (polycarbonate) shields do!
